Personalised Care Plans
We focus on tailoring healthcare and support services to meet the unique needs and preferences of individuals with disabilities through understanding each person's specific condition, lifestyle, and goals to provide customized interventions and resources, which promotes autonomy, improves quality of life, and ensures that care is responsive to the individual's physical, emotional, and social needs.
Community Inclusion
Actively involving participants in all aspects of community life, with focus on creating accessible environments, opportunities, and resources that enable participants to participate fully in social, economic, cultural, and political activities. which promotes equity, reduces isolation, and fosters a sense of belonging.
Supported Independent Living
Our Supported Independent Living (SIL) focus on providing tailored support to help individuals live as independently as possible in their own homes or community settings. These services can include assistance with daily activities such as personal care, meal preparation, medication management, housekeeping, and accessing community resources.
The level of support is customised to each participant’s needs, promoting autonomy and enhancing their quality of life. Our SIL aims to empower participants to achieve their personal goals, develop skills, and participate fully in their communities.
Community Nursing Care
Our Nursing care includes medication management (diabetes management, subcutaneous injections), wound care, bowel care, epilepsy management, urinary catheter care, tracheostomy support, health assessment, support and education. Our nurses also provide guidance and training to caregivers.
Mental Health
At Amion, we provide holistic support for individuals with complex mental health and physical disabilities, working collaboratively with you and community mental health services to address your needs.
We also assist in finding suitable accommodation and delivering tailored, personalised care aligned with your personal goals.
